New Client Registration
To become a TriCor customer, we need you to fill out these forms and fax them back to us. The forms are provided below in Adobe Acrobat (TM) format. Adobe Acrobat Reader (TM) , a free product, is required to view and print these documents and is also available below. One of the forms is the "Membership Application" which asks for basic information on your organization. The second form is the "User Agreement" which states that your organization will be in compliance with the Fair Credit Reporting Act regarding all aspects of the background screening process. This Agreement does not bind your organization to use TriCor for any length of time or number of searches. The third form is required if you will be requesting the social security number trace product. The fourth form is the Agreement for Multi-State Criminal Search. This form is only needed if you will be using the Multi-State Criminal Search Product. States that you understand the terms of the MSCS product.
